  Skilled Home Care (Skilled Nursing or Therapy)

Skilled home care services include professional nursing and therapy for adults and children who are recovering from a serious illness or injury, or who are disabled or chronically ill. It may also include assistance from a home health aide with activities like bathing and dressing.

Today, state-of-the-art medical equipment that can be used in the home and the qualities of highly-skilled staff, make it possible to provide treatment and services that were once only available in hospitals.

Professional Home Nursing’s health care team understands Home Care eligibility requirements and can determine if you qualify for skilled Home Care services.

Skilled home care is provided by licensed or certified health care professionals such as registered nurses (RNs); licensed practical nurses (LPNs); home health aides (HHAs); physical, occupational and speech therapists; and social workers. These professionals work in collaboration with your primary care physician to help you in a number of ways:

  Nursing

Nurse wrapping patient's woundRegistered Nurses evaluate health and provide case management services. Some of these services include injections and intravenous therapy, wound care, education on disease treatment and prevention, and patient assessments.  Provides you and your caregiver instructions to assist you to better take care of yourself and manage your disease.

 

 

  Physical Therapy

Physical Therapists (PTs) evaluate and treat to restore the mobility and strength of patients who are limited or disabled by physical injuries through the use of exercise, massage, and other methods. PT’s often alleviate pain and restore injured muscles with specialized equipment. They also teach patients and caregivers special techniques for walking and transfer.

  Occupational Therapy

Occupational Therapists (OTs) help individuals who have physical, developmental, social or emotionalOccupational Therapist with child problems that prevent them from performing the general activities of daily living (ADLs). OTs instruct patients on using specialized rehabilitation techniques and equipment to improve their function in tasks such as eating, bathing, dressing, and basic household routines.

 

 

  Home Health Aide

Home Health Aides are certified nursing assistants who are specially trained in Home Health Care. Home health aides assist patients with activities of daily living such as getting in and out of bed, walking, bathing, toileting, and dressing. Aides work under the supervision of a nursing professional.

  Speech Therapy

Speech therapists help people who have trouble swallowing or communicating due to a stroke or other illness. They provide professional evaluation and treatment of speech, hearing, and language disorders.

  Counseling      

Social workers evaluate the social and emotional factors affecting ill and disabled individuals and provide counseling. They also help patients and their family members identify community resources and can coordinate a variety of services.

 

We recognize that your home care needs are unique, so PHN’s Home Care team will individualize your plan of care under the direction of your physician. We will also assess your needs for other community resources and make referrals for additional assistance when appropriate.
